STEM Applications

ELEMENTARY STEM INSTITUTE APPLICANTS (rising Grades 4 & 5)  JUNE 3-7 ADD “ELEMENTARY STEM INSITUTE” IN THE TEXT FOR QUESTION #1

There are three on-line forms that need to be completed and submitted before an application may be processed. Only one form is submitted by the student applicant and parent. The other two are recommendation forms that are to be filled out by others: a teacher and a non-relative adult. Applicants are responsible for contacting a teacher and another adult to fill out the appropriate recommendation form. NOTE: STEM Institute students from previous years are not requried to provide new recommendations.

Student/Parent Form

• The student and parent must fill out and submit this form. This form is arranged in three parts: Student Information, Student Questionnaire and Parent Questionnaire. Parts of this form need to be completed by the student and other parts need to be completed by a parent.

Teacher Recommendation Form

• A teacher recommendation form must be filled out and submitted by a teacher.  The teacher will need your e-mail to link to your application.

Adult Recommendation Form

• A recommendation form must be filled out and submitted by an adult outside of the student applicant’s family.  Please provide your adult reference with your e-mail address to link the recommendation to your application.
